Quality Measures

CHADIS can assist with QI measures through offering reports, screens and diagnostic tools for the following Quality Measures:

 

Adult Major Depressive Disorder (MDD): Suicide Risk Assessment: Among other depression assessments, CHADIS provides access to a number of depression measures including Patient Health Questionnaire-9 Modified, Patient Health Questionnaire-9 Modified (PHQ-9-M CTS) and Mood and Feelings Questionnaire for adults.  Measure #107 (NQF 0104) Ages 18 and over.


Preventive Care and Screening: Screening for Depression and Follow-Up Plan:  CHADIS provides access to many depression screens and provides a graphical representation of the scores over time: PRIME MD PHQ (adult depression), Patient Health Questionnaire (PHQ-A)Patient Health Questionnaire-9 Modified, Patient Health Questionnaire-2, Mood Disorder Questionnaire (MDQ), Center for Epidemiological Studies Depression Scale for Children (CES-DC), Kutcher Adolescent Depression Scale, Pediatric Symptom Checklist – Youth, Mood and Feelings, teen and adult forms; long and short versions; and the Edinburgh Postnatal Depression Scale. Measure #134 (NQF 0418) Ages 12 and over.

Falls: Risk Assessment: CHADIS includes the Falls Risk Assessment Tool, a brief tool to assess falls risk in older people. Measure #154 (NQF 0101) 

 

Weight Assessment and Counseling for Nutrition and Physical Activity for Children and Adolescents:  CHADIS provides access to Brenner Families in Training (FIT) (nutrition/obesity and exercise). The Brenner FIT questionnaire asks parents a series of questions to assess the overall health and fitness habits of a child and their family plus questions about readiness for change. Also offered is the Healthy Habits Questionnaire Measure #239 (NQF 0024) Ages 3 - 17.


Dementia: Cognitive Assessment: CHADIS offers the Mini-Cog © a validated brief clinical screen for dementia. #281 Ages 18+

 

Adult Major Depressive Disorder (MDD): Coordination of Care of Patients with Specific Comorbid Conditions: In development. Measure #325

 

Maternity Care: Post-Partum Follow-Up and Care Coordination : CHADIS includes the Edinburgh Postnatal Depression Scale; Safe Environment for Every Kid (SEEK); SEEK-PLUS (F/U with barriers); and the Family Assessment of Safety and Stress (FASS) screens for Social Determinants of Health- if positive computer begins motivational interview (MI)  triggering MI “teleprompter” language for clinicians; patient messaging and options for connection to local resources  Measure #336

 

ADHD: Follow-up Care for Children Prescribed ADHD Medication: CHADIS offers the ADHD Patient Specific Template (PST), which gathers pre-visit ADHD information from parents AND teachers without any clinician effort. 

 

The pre-visit ADHD-related questionnaires cover all essential questions for an ADHD check-up visit. This includes parent and teacher Vanderbilts, screening for DSM disorders, and more. School data is essential to optimizing ADHD care and medications. Teachers can be “invited” by parents or staff to complete Vanderbilts via CHADIS with FERPA compliant online parent consent forms.  The complete set of Conners questionnaires; preschool and adult ADHD questionnaires are also available.

 

Each PST provides a tailored graphic to be shown to families, illustrating the patient’s ADHD challenges over time, facilitating discussion. The PST suggests tailored, research-supported, goals for clinicians to review with parents. Parents are sent specific suggestions for how to accomplish the chosen goals. Selected goals are also listed in subsequent PSTs, so clinicians are reminded to follow-up on progress. The 504 plan and IEP letters can be automatically generated as well as post-visit messages tailored to the parent’s goals and challenges. Printable handouts and local resources are all accessible to the parent online in the CHADIS MemoryBook Care Portal. Measure #366 (NQF 0108) Ages 6 - 12.

 

Bipolar Disorder and Major Depression: Appraisal for alcohol or chemical substance use: CHADIS offers Substance Risk Estimator, CRAFFT©,  DEP-ADO and  Version 3.2 (DEP-ADO). Measure #367 (NQF N/A)

 

Depression Remission at Twelve MonthsCHADIS offers the Patient Health Questionnaire-9 Modified and Patient Health Questionnaire-9 Modified (PHQ-9-M CTS). In addition, The Mood and Feelings-parent and teen; long and short forms with graphic representation to demonstrate clinical progress Measure #370 (NQF 0710) Ages 18 and older.

 

Depression Utilization of the PHQ-9 Tool: CHADIS provides access to Patient Health Questionnaire-9 Modified and Patient Health Questionnaire-9 Modified (PHQ-9-M CTS). Measure #371 (NQF 0712) Ages 18 and older.

Maternal Depression Screening:  CHADIS has the Edinburgh Postnatal Depression Scale which is one of the most widely-used tools for the assessment of postpartum depression. The Family Assessment of Safety and Stress (FASS) also includes parental depression with decision support and connection to local mental health services as an option. #372 Ages 0-6 months postpartum.

Child and Adolescent Major Depressive Disorder (MDD): Suicide Risk Assessment: CHADIS provides access to Ask Suicide Screening Questions (ASQ) and Patient Health Questionnaire-9 Modified (PHQ-9-M CTS). Measure #382 (NQF 1365) Ages 6 - 17.

Optimal Asthma Control: CHADIS Asthma Questionnaires include Asthma Control Test 4-11 & 12+ versions and Pediatric Asthma Control & Communication Instrument (PACCI). Note that the Asthma Care Patient Specific template has been shown to result in improved asthma control in 100% of patients significantly improved over controls in a national study. Depression Remission at Six Months:  CHADIS provides access to Patient Health Questionnaire-9 Modified and Patient Health Questionnaire-9 Modified (PHQ-9-M CTS). Measure #411 (NQF 0711) Ages 18 and older.

Preventive Care and Screening: Unhealthy Alcohol Use: Screening & Brief CounselingCHADIS offers the Alcohol Use Disorders Identification Test-Consumption (AUDIT-C) and assists in the screening portion of this quality measure. #431 (NQF 2152)

 

Medication Management for People with Asthma: CHADIS proactively manages chronic and preventive care using evidence-based protocols to guide treatment of asthma through the use of the Asthma Care Patient Specific Template which has been shown through a national cluster randomized trial to improve asthma outcomes in children. Clinicians gather on-line, pre-visit asthma information from the patient, receive tailored clinical ‘Next Steps’ to easily follow during the visit, receive "problem solving counseling" suggestions to address adherence, efficiently create an individualized Asthma Treatment (Asthma Action) Plan with the patient/family, receive tailored resources for the patient/family, and have access to clinician education all while earning Part 4 MOC credits. All of the data, information, and plans are then documented within your EHR.

Developmental Screening in the First Three Years of Life: CHADIS includes Ages & Stages (ASQ & ASQ-3); Child Development Inventory (CDI); Infant Development Inventory; SWYK; and other tools that meet Bright Futures Guidelines. #467 (NQF 1448) Ages 1-3. Measure #467 (NQF 1448).
