CHADIS is pleased to announce a new release, scheduled to be available as of November 10, 2019. The release contains feature enhancements and new questionnaires. Please review this information and the attachments, which can benefit your practice and your patient care.
New Feature for Offices!
Clinical user management: You asked, we listed! CHADIS now allows practice administrators to completely manage clinical users. This means that clinical users can be added, edited, and disabled/removed by all practice level administrators! These new features are available via the “Office Administration” button on the CHADIS home page:

This initial stage of the Help Center primarily includes support for standalone users. We'll be adding new topics moving forward.

Role Name Changes
We have clarified the names of user roles.
CHADIS standalone clients: Create QR Codes for Your Office Invitations
Simply post these QR Codes in your office, so that new respondents can scan the code with their smartphone and jump directly into the CHADIS registration process, with your office invitation code pre-filled. Note: If your EMR is integrated with CHADIS, parents and patients do not self-register with an invitation code.
Instructions: From your home screen click the 'Office Administration' button > 'Invitations' > Click on the 'QR Code' button next to any office invitations you have created.
New questionnaires: The following questionnaires were recently released to all offices. Please review the descriptions below. Please click “View Questionnaires” from the CHADIS Homepage in order to review the questionnaire metadata and preview the questions:
Oral Risk Assessment: The American Academy of Pediatrics (AAP) has developed this tool to aid in the implementation of oral health risk assessment during health supervision visits. This tool has been subsequently reviewed and endorsed by the National Interprofessional Initiative on Oral Health.
Maryland EPSDT Forms: MD EPSDT Combined, MD EPSDT Heart Risk, MD EPSDT – Fluoride, MD EPSDT – Lead, EPSDT – Tuberculosis - These questions are recommended/required for use of the providers when doing preventive visits per Healthy Kids Program guidelines in Maryland.
Family and Adult Medicine Questionnaires
CHADIS Medicare Annual Wellness Questionnaire: This questionnaire was designed to cover all patient-report topics for a standard Medicare Adult Wellness Visit.
STEADI (Stopping Elderly Accidents Deaths & Injuries): A 12 item validated fall risk self-assessment tool.
WHODAS 2.0 Questionnaires (World Health Organization Disability Assessment Schedule 2.0): The WHODAS assesses disability across six domains, including understanding and communicating, getting around, self-care, getting along with people, life activities (i.e., household, work, and/or school activities), and participation in society.

More information will be shown when you view invitations, including the relationship name (e.g. teacher) and name of individual who used the invitation.
When viewing a respondent, you will be able to see the list of all patients related to them, and their relationships.
In order for the system to be upgraded, CHADIS will be unavailable from 9 am ET to 11 am ET on Sunday, November 10th.
