Question  Was the COVID-19 pandemic associated with changes in children’s developmental milestone attainment as assessed in US pediatric primary care practices?

Findings  Using an interrupted time series design in this cohort study of 50 205 children, adjusted age-standardized mean Ages and Stages Questionnaire screening scores showed modest decreases from the prepandemic period to the intrapandemic period for communication, problem-solving, and personal-social domains. There were no changes in fine or gross motor domains.

Meaning  Modest decreases in developmental screening scores suggest reason for cautious optimism about the development of a generation of US children exposed to the COVID-19 pandemic.
