How to Use CHADIS - For Families

Unless your doctor's office has different instructions, follow the steps below.

 

First-time Users will need an invitation code from their doctor's office prior to using CHADIS. (Usually the doctor's office phone number without dashes)

Or,

• Go to CHADIS.com  

• Click Register in the top right corner of the website.

Returning Users do not need an Invitation code, you can just enter the username and password you created the first time you logged into CHADIS.

• Click Login in the top right corner of the website.

Note: The CHADIS website and system operate with the highest level of security and ALL patient information is completely confidential. Only your healthcare provider and their staff can see it.
